2025 Madison Boys High School Soccer Team Camp Information: By paying you are also registering. Payment = Registration and consent to the legal rights agreement and waiver(s) contained herein.

Early Payment Deadline is March 15, 2024: $569.50
Late Payment Deadline is April 15, 2024: $619.50

The price will increase $50.00 on March 16th, 2025. Registrations made after March 15 are considered late and there will be no guarantee of acceptance. The late fee registration is an additional $50 over the original price. Please pay your money by the deadline(s). We will collect paperwork and forms from you the day we leave for camp.

MHS Boys 2025 Team Summer Soccer Camp; June 16-20, Price Utah, Utah State University Eastern Campus. We leave Monday morning (June 16) around 2:00 a.m. We return Friday evening Cost will be $544.50 plus $25 Van rental and fuel cost for a total of $569.50 per player. Deadline to register will be March 15, 2025 by midnight. All players (incoming 9th to 12 graders strongly encouraged to attend).

If made after March 15, 2024 the price will increase. No cash or checks accepted for this camp - NO EXCEPTIONS. We have already had to place a large deposit six months in advance.

It will be a RESIDENTIAL camp (e.g. Players will stay on campus or something similar during the night). There will be some day-trip travel throughout the region for activities related to team camp. Details to be provided. 

This is open to any 9-12 grader.

Camp fees will include lodging and meals and transportation to and back from Price, Utah and admission to the team camp. Breakfast the day of travel to Utah and dinner the day returning from Utah is NOT included. Please plan accordingly.

 Legal - By Paying/Registering, I hereby agree to the terms set forth herein and below. MADISON SCHOOL DISTRICT 321 TRAINING, DEVELOPMENT, COACHING, AND/OR SOCCER CAMPS (Assumption of Risk; Waiver of Liability; Release; Indemnification; Covenant Not to Sue) Acknowledgement of Risk: I (meaning a parent/guardian of a minor child participant) on behalf of the minor, understand and acknowledge that participating in the MADISON SCHOOL DISTRICT 321 SOCCER TRAINING, DEVELOPMENT, COACHING, AND/OR SOCCER CAMP (herein “Activity”), entails both known and unanticipated risks related to this Activity, which include, but are not limited to: traveling on the interestate, highways, and roads to and from Merdian, Idaho and the surrouding areas during camp, broken bones, sprains, strains, dehydration, concussion, paralysis, and even death, as well as damage to property or third parties, allergic reactions to food or drink, or other unknown and unanticipated activities and risks. It is acknowledged that a team paintball activity maybe part of this camp. It is acknowledged that the team will travel to and from Meridian and that coaches will drive the kids in a large van or vehicle(s). I certify that on behalf of my minor child, I am aware of these voluntarily assumed risks; that my minor child is in good health; and has no physical or mental limitations that would preclude safe participation.

Release/Indemnification/Covenant Not to Sue: To the fullest extent permitted by law, and in consideration for being allowed to participate in this Activity, I, on behalf of my minor child, my heirs, representatives, executors, administrators, and assigns (the Releasing Parties) hereby agree to hold harmless, release, and covenant not to sue MADISON SCHOOL DISTRICT 321 its members, respective officers, employees, volunteers, and agents, (the Released Parties) for any negligently caused injuries or losses arising from or related to the Activity. I further agree to defend and indemnify the Released Parties and each of them from any claims, demands, actions, damages, costs, fees, or expenses arising out of losses suffered by or caused by my minor child that are brought now or in the future by the Releasing Parties or any of them, or by a third party.

Other: I acknowledge that insurance coverage for bodily injury and property damage is my personal responsibility. On behalf of my minor child, I hereby give permission for emergency medical care, including transportation to and exchange of medical information with a medical facility. I grant MADISON SCHOOL DISTRICT 321 the right to use, for promotional purposes, any photographs or video footage taken of my minor child during this Activity. This agreement and all claims arising out of or relating to this agreement shall be governed exclusively by the laws of Idaho. The disputes shall be resolved in the courts of Madison County, Idaho, and I, on behalf of my minor child, consent to the jurisdiction of such courts. By registering/paying, I am electronically executing this document as if it had been signed in paper as a parent/guardian of a minor child and represent and warrant that I have the legal right to execute this waiver on behalf of the minor and that the release, once executed by me, is fully enforceable in accordance with its terms. I agree to indemnify the Released Parties in the event the representation is not accurate.

By registering/paying, I am electronically executing this document as if it had been signed in paper as a parent/guardian of a minor child and represent and warrant that I have the legal right to execute this waiver on behalf of the minor and that the release, once executed by me, is fully enforceable in accordance with its terms. I agree to indemnify the Released Parties in the event the representation is not accurate. TIME IS OF THE ESSENCE IN THIS AGREEMENT AND I AND MY HEIRS HEREBY INTEND TO BE BOUND BY THE LAWS OF THE STATE OF IDAHO.
